Rantoul City Schools anticipates partnering with RTHS and OSF to provide a vaccination clinic for students 12 years old and older on May 19 and May 20. This survey is intended to gauge your interest in having your student receive the COVID vaccine during this vaccination clinic. Regardless of your responses to this survey, additional information will follow, including the written consent form.
