The Champaign-Urbana Schools Foundation (CUSF) is accepting nominations for its 2016 Distinguished Alumni Awards through Dec. 31.
The award honors individuals who graduated from Champaign Unit 4 and District 116 high schools and have made great contributions to society through their achievements, character and citizenship. To be considered for 2016, nominees must have graduated from Champaign High School, Champaign Central High School, Champaign Centennial High School or Urbana High School in 2006 or earlier.
The nomination form includes general information about the nominee such as date of birth and address, a personal statement of recommendation, and a vita or resume. Nominators are also encouraged to submit a black and white photo of the nominee. They should also include, if relevant, additional information on the nominee such as civic and philanthropic activities, military service, professional and commercial work, non-business directorships and trusteeships such as foundations, and previous recognition.
Completed nomination forms can either be mailed or emailed to the CU Schools Foundation.
This year's alumni winners, as well as past ones, from Central and Centennial High Schools will receive a plaque that will be on display at both school's newly established Distinguished Alumni Halls of Fame.
