Champaign Urbana Nursing & Rehab, a 213-bed skilled care facility in Savoy, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the first quarter of 2019.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Champaign Urbana Nursing & Rehab was cited with an A violation for an incident in November 2018 and fined $25,000.
