Champaign Living Center, a 102-bed skilled care facility in Champaign, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the first quarter of 2019.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Champaign Living Center was cited with two A violations for incidents in February and fined $50,000.
