Champaign Urban Nursing and Rehab, a 213-bed skilled care facility in Savoy, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the third quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Champaign Urban Nursing and Rehab was cited with an A violation in July and fined $25,000.
