C. UIUC Evaluation Capacity Proposal* (pages 38-43)

D. 2-1-1 Information and Referral* (pages 44-45)

A Decision Memorandum requests renewal of CCDDB and Champaign County Mental Health Board (CCMHB) involvement with the United Way of Champaign County {UWCC) in support of the 211 information and referral call service. A copy of the new contract between UWCC and PATH is included. Board action is requested.
