Champaign Community Unit School District No. 4 Board of Education issued the following announcement on June 12.
All entering Kindergarten, 6th grade and 9th grade students are required to have proof of a physical and immunizations. Students entering 12th grade have a vaccine requirement only. Learn more about the requirements and forms needed.
All forms must be submitted by September 1, 2018.
Students who have not submitted a school physical form by September 1 will be excluded from attending school until all required examinations and vaccinations have been completed.
The providers listed below are offering physicals and immunizations.
Carle
Schedule a school physical by calling (217) 383-3200, Monday through Friday, 7 a.m. to 6 p.m. Or, if you are an existing Carle patient, you may schedule online using your MyCarle account.
Special extended hours will be available on the following days:
August 7, 8, & 9 - Urbana on Windsor, 6 – 8 p.m.
August 14, 15, & 16 - Urbana on Windsor, 6 – 8 p.m.
You must call (217) 255-9500 to schedule an appointment during the extended hours at Urbana on Windsor above. Walk-ins not accepted.
Child must be accompanied by a parent or legal guardian. If possible please bring any pre-completed, required forms and immunization records.

Champaign-Urbana Public Health District
Physicals and immunizations are offered Monday through Friday between 8:30 a.m. and 3:30 p.m. Must call for an appointment. We may be able to accommodate clients at other times, including the third Saturday in June and July of 2018. Please contact the nurse at (217) 531-4317 or (217) 531-4301.
Christie Clinic
Schedule a school or sports physical by calling your provider. A school physical clinic will also be offered Saturdays June 30 - July 28, 9 a.m. - noon. These will require registration and the child must be accompanied by a parent or legal guardian. If possible, please any pre-completed, required forms and immunization records. Call Pediatrics at 217-366-1257 to make an appointment.
